Public Health Nurse Case Management
Family‑centered case management services are delivered remotely for children who are lead-poisoned within the lower blood level range of 3.5 to 9.4 mcg/dL. Services include lead and nutrition education, nursing assessment to assist in identifying the source of exposure, an individualized nursing care plan, and care coordination. Additionally, we offer consulting and training for healthcare providers as a collaborative effort to improve health outcomes for children. Services are available under government contracts (county/state or FQHC), as well as private‑pay options for families seeking specialized guidance for their child’s healthy development.
